Sun Health debuts program to improve patient care, tracking

Sun Health announced today that it has improved its Care Transitions program, which serves patients discharged from Banner Boswell Medical Center in Sun City and Banner Del E. Webb Medical Center in Sun City West.

The Care Transitions program, which was created to improve patient care, reduce readmissions and improve collaboration among providers, will be powered by the Care Team Connect web-based platform.

Care Team Connect’s secure, web-based platform connects hospitals, community providers, family members and patients so they can work in partnership to improve patient outcomes while reducing the overall cost of care, officials said.

Based on the needs of each patient, Care Team Connect generates a patient-specific care plan and improves communication among all parties involved in patient care.
